Pianos are restored inside and out. It can be partially restored to meet your budget.
If the instrument is primarily used as a decorative ornament, the exterior of the piano is the first thing that people notice and it is refinished.
If people are actively playing it, sound is important. The mechanical guts of the piano that make the sound gets restored. That entails wooden parts, hammers, strings, keys, pins and there are many uses of felt in parts of the action. There is a lot to it.
This particular Vose & Sons Grand Piano is in the process being restored. If you look at the images below you can see how time makes a beautiful piano unrecognizably ugly. The same beautiful piano is still there ... you just can't see it. Restore it and it's like new, gorgeous. And the great sound comes back.

The keyboard alone has hundreds of parts. Each key has more than 20 parts, including fasteners. Key mechanisms are made of wood, felt, springs, and fasteners. It is a labor of love, with an accent on the labor. However, a minor broken part can be fixed on a service call, without getting into a big restoration project.
